Appointments are available online via telehealth in-person and at our Scottsdale office. New patients are welcome!
Sliding fee scale is also available for those who qualify. Additional carriers may be accepted. To receive an accurate cost estimate including any applicable co-pays, please fill out and submit a patient intake form. We are able to provide a super-bill to those who are out of network. At this time we are not currently in-network with Medicaid or Medicare, even if they are contracted with one of the above insurers.
